How to refer

Referrals can be made by families, carers, support coordinators, schools, or other health professionals. NDIS funding under Improved Daily Living or Capacity Building can be used.

Individuals experiencing anxiety, depression, trauma, grief, adjustment difficulties, or those requiring psychological assessment for disability, NDIS, or aged care purposes.

How to refer

Self-referrals welcome. GP referrals under a Mental Health Care Plan may attract Medicare rebates. NDIS funding under Improved Daily Living can also be used.

A key worker is a dedicated professional who acts as the central point of contact for individuals and families navigating multiple services. They coordinate care, build relationships, and ensure that supports are working together — reducing the burden on families and improving outcomes.

Families of young children with developmental concerns or disability, NDIS participants with complex support needs, and individuals who need help navigating the service system.

How to refer

Referrals can be made by families, early childhood services, or other professionals. NDIS funding under Early Childhood or Coordination of Supports may apply.

Children with developmental delays, adults with disability or injury, and older Australians experiencing changes in function due to ageing, illness, or cognitive decline.

Referrals accepted from individuals, families, GPs, hospitals, and support coordinators. NDIS funding under Improved Daily Living or Capital Supports (for equipment/modifications) can be used.
